Fall Season 2009 - 2010 Registration Information

 

 

 
To drop a player from team, a COMPETED DROP FORM MUST BE signed by the Parent and the Division Coordinator. The COMPLETED DROP FORM MUST CONTAIN:
 
1. PARENT’S/GUARDIAN’S ME, ADDRESS, AND SIGNATURE
2. DIVISION COORDINATOR’S SIGNATURE
 
The Drop Form MUST BE GIVEN to the Division Coordinator. You can obtain a DROP FORM from this page. The COMPETED DROP FORM AND A SELF-ADDRESSED STAMPED ENVELOPE can also be mailed to the Regional Registrar, P.O. Box 1517, Ontario, CA, 91762.
 
A refund will be paid in 4 to 6 weeks after the COMPLETED DROP FORM is received by the Regional Registrar.

HELP! MY CHILD HAS NOT BEEN CONTACTED BY A COACH YET?
 Right now Coaches are being trained and certified or we may still be looking for a volunteer to step up for your child’s team. All teams have been formed and coaches cannot contact any player until they have finished their training and certification. Some teams are starting practices now. Coaches have until August 26th to contact players. After that date if you have still not been contacted please notify the region hotline at 909-391-4199 ext 4 or email wgomez22001@yahoo.comand leave the following information:
Your Childs/Players Name.
Your Childs/Players Date of Birth
Your Name
Your Contact Phone Numbers
Your email
We will Contact your Division Coordinator and they will contact you to get you in touch with your coach.
